1.) Proof of membership to at least one of the following associations:

• ASID (American Society of Interior Design)
• IIDA (International Interior Design Association)
• AIA (American Institute of Architects)
• IDS (Interior Design Society) 
• CID (Certified Interior Decorators International)
• DSA (Designer Society of America)

2.) Photo ID (driver’s license or passport).

3.) A completed W9 form.

4.) Business License or Sales Use Tax Permit or Certificate of Incorporation/Organization.
